[Dar-us-Salaam] Your Feedack Needed: DUS Family Medical Practice

Assalamu Alaikum. The DUS Family Medical Practice is one of the
projects of the Dar-us-salam Community. It is 3 ½ years old and
alhamdulilAllah maintains over 6 thousand patients with 70 new
patients coming in every month. To serve the community better the
Family Medical Practice is conducting a survey to assess the
community's medical needs. The survey will take approximately 5
minutes; please answer the questions honestly as your responses will
remain anonymous and undisclosed. It is extremely important that you
take this survey to help us to improve and help the community better,
